Welcome to the Clackamas County Public Health Birth and Death Certificate online ordering system. Requests will be processed from 7:00 AM - 5:30 PM on Monday through Thursday.
If you need a birth or death certificate for an event that occured less than six months ago in Clackamas County, please follow the steps outlined below. If the event was more than six months ago or in a different county, order through the Oregon Health Authority at 971-673-1190. For more information, please contact the Vital Records office at 503-655-8406.
Birth Certificates
- By law, only immediate family members or legal representatives may order birth certificates for another individual.
- Most people can expect to receive their certificate within a few weeks, although in-person requests could yield the certificate the same day.
- Birth certificates are issued at the Vital Records office during the following dates and times:
- Monday - Thursday: 7:00 AM - 5:30 PM
Death Certificates
- Funeral Home Instructions:
- If you work for a funeral home, please enter your personal name in the requestor fields during the application process.
- Please enter the name of the Funeral Home itself in the special instructions field during step 4 of the application process.
- By law, only immediate family members or legal representatives may order death certificates for another individual.
- Death certificates are issued at the Vital Records office during the following dates and times:
- Monday - Thursday: 7:00 AM - 5:30 PM
